Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to an age-appropriate, culturally relevant plan of care for adult and geriatric residents; involve the resident/family and healthcare team; promote a safe environment.
- Recommend revisions to the plan of care after evaluating resident outcomes and coordinate referrals to address all aspects of care.
- Collaborate with residents, families, and the Interdisciplinary Team; participate in continuing education and in-service activities; mentor and orient other staff as appropriate.
- Provide leadership within the professional practice setting under RN direction; maintain current nursing knowledge and competency; adhere to safety and infection control guidelines.
- Recognize potential exposure to bloodborne pathogens and follow standard precautions to minimize risk.

Requirements
- NY LPN licensure; active and in good standing
- Basic Life Support (BLS) certification (certification through AHA or RQI specifically)
- Minimum 1 year of experience as an LPN in a long-term care facility
- Ability to provide care for adult and geriatric residents
- Weekend and holiday availability required
- Demonstrated ability to work under RN supervision in a skilled nursing setting
