Responsibilities
- Conduct patient evaluations and re-evaluations to determine OT goals and treatment plans.
- Implement and adjust evidence-based OT interventions to improve activities of daily living, mobility, balance, strength, and coordination.
- Utilize modalities and therapeutic techniques appropriate to an acute rehabilitation or SNF setting; monitor progress and outcomes.
- Document patient progress, treatment plans, and discharge recommendations accurately in clinical records.
- Collaborate with an interdisciplinary team (PT, nursing, speech therapy, social work) to optimize care and coordinate discharge planning.
- Educate patients and families on home exercise programs, safety strategies, and durable medical equipment needs.

Requirements
- Active Certified Occupational Therapy Assistant (COTA) license and credentialing appropriate to practice in the United States; state-specific requirements as applicable.
- Graduation from an accredited OTA program.
- Experience in rehabilitation settings; SNF or acute care experience preferred.
- Strong clinical documentation, communication, and teamwork skills.
- Ability to work in a travel assignment with a 5x8 day shift schedule; comfortable operating in a fast-paced environment.
