Responsibilities
- Conduct comprehensive speech, language, cognitive-communication, and swallowing assessments for residents within the care setting.
- Develop and implement individualized treatment plans, including therapy for speech, language, voice, cognitive-communication deficits, and dysphagia management; monitor progress and adjust plans as needed.
- Provide direct therapy and supervise care plans in collaboration with an interdisciplinary team (nursing, OT, PT, dietitian, social work) to support functional communication and safe eating and swallowing.
- Perform appropriate documentation including evaluations, progress notes, treatment plans, and discharge summaries in accordance with regulatory and facility standards.
- Participate in care planning and discharge planning, providing education to residents, families, and staff to support ongoing communication strategies and safe swallow practices.
Requirements
- Active Connecticut state license as a Speech-Language Pathologist (SLP)
- Master’s degree in Speech-Language Pathology from an accredited program
- ASHA Certification (CCC-SLP) preferred or eligible
- Clinical experience in speech-language pathology, with emphasis on acute care or long-term care settings preferred
- Ability to work in a long-term care environment and collaborate with an interdisciplinary team
- Eligibility to work in the United States
