Responsibilities
- Perform initial PT evaluations and develop individualized treatment plans for residents
- Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
- Reassess progress, adjust plans of care, and document outcomes regularly
- Maintain accurate, compliant documentation in resident records and care plans
- Collaborate with an interdisciplinary team (nursing, therapy, social services) to optimize care
- Participate in discharge planning and provide education to residents and families as appropriate
Requirements
- Active Physical Therapist license (status to be verified during hiring)
- Ability to provide physical therapy services in a skilled nursing facility setting
- Experience in geriatric rehabilitation or SNF environments preferred
- Strong communication and documentation skills
- Willingness to commit to a 13-week travel assignment with guaranteed hours
