Responsibilities
- Conduct formal OT evaluations and develop individualized treatment plans for residents
- Implement evidence-based therapy to improve activities of daily living, mobility, upper-extremity function, and cognitive engagement as appropriate
- Reassess residents regularly; adjust treatment plans and document progress notes in the chart
- Collaborate with nursing, physical therapy, speech, social work, and other team members to coordinate resident care
- Educate residents and families on home programs, safety, and adaptive strategies
- Ensure documentation accuracy and compliance with facility policies and regulatory requirements
Requirements
- Active California Occupational Therapist license in good standing
- Master’s or Doctorate degree in Occupational Therapy from an accredited program
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
- Strong assessment, treatment planning, and documentation skills
- Ability to work effectively in a fast-paced, multidisciplinary SNF environment
- Willingness to commit to a 13-week travel assignment starting May 18, 2026
