Responsibilities
- Provide direct nursing care to residents, including monitoring health status and administering medications within the scope of practice.
- Document care in the PCC (PointClickCare) Electronic Health Record, updating assessments, care plans, and progress notes.
- Monitor vital signs, manage routine nursing tasks, and respond to changes in clinical status according to facility policy.
- Collaborate with the interdisciplinary team to implement and adjust individualized care plans; communicate with families as appropriate.
- Assist residents with activities of daily living and support wound care and infection control protocols as needed.
Requirements
- Current LPN license in good standing (state requirements may vary by assignment)
- Proficiency with PCC (PointClickCare) or a comparable EHR system
- Ability to work a mixed schedule including 12-hour night shifts (7:00 PM–7:00 AM) and 8-hour evening shifts (3:00 PM–11:00 PM)
- Experience in a long-term care or skilled nursing setting preferred
- Willingness to rotate across all units within the facility
