Responsibilities
- Provide direct patient care to residents under supervision of an RN, in a long-term care setting
- Administer prescribed medications and document administration accurately
- Monitor vital signs, assess patient conditions, and report changes promptly to the care team
- Assist residents with activities of daily living and support activities that promote safety and independence
- Participate in care planning, nursing assessments, and documentation in electronic health records
- Maintain infection prevention protocols, accurate charting, and a safe, respectful care environment
Requirements
- Active LPN license in the state of New Hampshire
- Minimum 1 year of practical nursing experience in long-term care or similar setting (preferred)
- Ability to work a 12-hour shift pattern: 07:00–19:30, on a travel assignment
- Strong communication and teamwork skills; ability to document clearly and collaborate with the care team
- Willingness to travel and comply with standard background checks and onboarding requirements
