Responsibilities
- Conduct comprehensive OT evaluations, establish goals, and design individualized treatment plans for SNF residents.
- Implement evidence-based interventions to improve activities of daily living, mobility, cognition, and upper-extremity function; provide adaptive equipment recommendations as appropriate.
- Document assessments, treatment plans, progress notes, and discharge planning in the resident’s chart per department standards and regulatory requirements.
- Collaborate with the interdisciplinary team, including families and caregivers, to support resident goals and participate in care planning meetings.
- Reassess resident progress regularly; adjust goals and interventions; support safe transfers and discharge preparation.
- Maintain compliance with infection control, safety protocols, and facility policies during therapy sessions.

Requirements
- Active Occupational Therapist license in Massachusetts or eligibility for licensure
- Degree from an ACOTE-accredited OT program (Master’s or Doctorate)
- NBCOT certification or current eligibility
- Experience in a skilled nursing facility or long-term care setting preferred
- Ability to work a 13-week travel assignment and comply with the 08:00–16:30 schedule
- Strong documentation and communication skills; ability to collaborate with an interdisciplinary team
