Responsibilities
- Provide direct nursing care to residents in a LTC setting, including assessment, monitoring, and documentation of health status.
- Administer medications and treatments per physician orders; monitor for adverse reactions and document administration.
- Develop and follow care plans; update records in the electronic medical record and communicate changes to the care team.
- Recognize changes in resident condition, implement appropriate interventions, and escalate to the supervisor or physician as needed.
- Collaborate with the interdisciplinary team (nursing, therapy, dietary, social services) to coordinate care and ensure quality resident outcomes.
- Maintain safety and infection control standards; support dementia care needs and assist with activities of daily living as required.
- Be prepared to float to other units to meet unit and patient care needs; adhere to unit policies and resident rights.
Requirements
- Active Registered Nurse license in Vermont (or eligibility in-state) with licensure in good standing.
- BLS through American Heart Association (AHA) required.
- Minimum 3 years of nursing experience; LTC experience strongly preferred; prior travel experience a plus.
- Ability to float to other units as needed; comfortable caring for residents with dementia.
- Willingness to complete a comprehensive background check: 7-year nationwide background check (including county court searches, FACIS3, and sex offender) within 18 months of start date; FACIS-3 and VCIC within 30 days of start.
- Reliable transportation to and from assignment; no explicit block scheduling; ability to work a 3x12-hour shift pattern and a flexible weekly schedule.
