Responsibilities
- Provide direct patient care to residents under supervision, delivering basic nursing care and monitoring status.
- Administer prescribed medications and treatments in accordance with policy; document accurately.
- Monitor vital signs, assess changes in condition, and escalate to the supervising RN as needed.
- Assist with activities of daily living and provide comfort and support to residents and families.
- Maintain accurate nursing documentation and ensure compliance with infection control and safety standards.
- Collaborate with the interdisciplinary team to implement and update individualized care plans.
Requirements
- Active LPN license in good standing (state of licensure appropriate to assignment)
- Current CPR/BLS certification
- 1+ year of practical nursing experience; prior long-term care experience preferred
- Ability to work a 7:00 a.m.–3:30 p.m. shift; 40 hours per week; local candidates preferred
- Strong communication, assessment, and documentation skills
