Responsibilities
- Deliver clinical nutrition assessment, planning, and ongoing monitoring for residents and patients; develop individualized care plans and adjust as needed.
- Provide nutrition counseling and education to residents, families, and multidisciplinary teams; support therapeutic diets and menu modifications as prescribed.
- Collaborate with physicians, nurses, and foodservice staff to implement nutrition interventions and optimize care delivery.
- Maintain documentation and communication with care teams; participate in care coordination, discharge planning, and quality improvement initiatives.
- Ensure compliance with The Joint Commission, New York State Department of Health, CMS, and other regulatory requirements; stay current with relevant guidelines and standards.
- Serve as a liaison between providers, nursing staff, and foodservice to support accurate nutrition care and safe food service operations.

Requirements
- Active Registered Dietitian (RD) license or eligibility; current registration to practice
- BLS certification
- Master’s degree preferred
- Prior clinical nutrition experience in long-term care and/or hospital settings preferred
- Ability to work on-site in the listed location and support cross-site clinical needs as part of a travel assignment
