Responsibilities
- Provide direct patient care to adults 60+ in a post-acute long-term care setting
- Administer medications and perform routine nursing tasks within scope of practice
- Document in the electronic medical record (EPIC) and communicate care plans with the interdisciplinary team
- Support activities of daily living, safety, and comfort; monitor for changes in condition
- Perform phlebotomy, EKGs, transports, and other unit procedures as allowed; assist with starting IVs where permitted
- Collaborate with RNs, CNAs, and other staff to ensure smooth workflow and high-quality care
- Respond promptly to call lights and participate in unit rounds and care planning
- Adhere to dementia-capable care standards when applicable and provide patient-family education as needed
Requirements
- Licensure: Vermont LPN license required at time of submission
- Certifications: BLS required; IV certification preferred for LPNs
- Experience: Minimum 1 year of clinical experience; prior travel experience required
- EMR: EPIC experience within the past 2 years required
- Education: Standard practical nursing education; appropriate state licensure
- Additional: Dementia-capable care certification preferred; ability to work in a post-acute long-term care unit with geriatric patients
- Credentialing: VT license confirmation; 7-year nationwide background check; FACIS 3 and VCIC within stated timelines
