Responsibilities
- Perform comprehensive OT assessments and establish individualized treatment goals for residents with functional limitations related to activities of daily living, mobility, and cognitive/ perceptual deficits.
- Plan, implement, and monitor evidence-based OT interventions to improve independence in activities of daily living, mobility, gait and balance, upper-extremity function, and cognitive skills.
- Document evaluations, progress notes, treatment plans, and discharge summaries in accordance with regulatory and facility standards; maintain accurate, timely records.
- Collaborate with an interdisciplinary team including nursing, physical therapy, speech therapy, social work, and physicians to coordinate care and optimize outcomes; participate in care planning meetings.
- Educate residents and families on therapy goals, home exercise programs, safety strategies, fall prevention, and adaptations to living environments; provide caregiver training as needed.
- Ensure compliance with risk management, infection control, and safety guidelines; monitor for adverse events and adjust interventions accordingly; participate in quality improvement initiatives.
Requirements
- Current, valid Occupational Therapist license to practice in Washington state or eligibility to obtain licensure
- Degree in Occupational Therapy from an accredited program
- Minimum 1 year of OT experience in a skilled nursing facility or geriatric setting preferred
- Ability to work in a fast-paced skilled nursing facility environment and manage a caseload
- Strong communication and documentation skills
- Availability to work the stated shift and guaranteed hours
