Responsibilities
- Evaluate residents’ functional status and mobility; develop and implement PT treatment plans aligned with overall care goals
- Provide individual therapy focused on gait training, transfers, strengthening, balance, range of motion, pain management, and functional restoration
- Document evaluations, progress notes, treatment plans, and outcomes; maintain accurate records and communicate with the interdisciplinary team
- Educate residents and caregivers on home exercise programs, safety, fall prevention, and activities of daily living (ADL) strategies
- Collaborate with therapists, nursing staff, and other teammates to ensure safe, effective care and coordinated discharge planning
- Adhere to professional standards, maintain patient safety, privacy, and infection control practices
Requirements
- Active Physical Therapist license in Montana or eligibility to practice
- Degree in Physical Therapy from an accredited program (DPT preferred)
- Prior experience in geriatric or long-term care settings preferred
- Strong assessment, documentation, and communication skills
- Ability to work effectively in a nursing home/long-term care environment
- CPR/BLS certification preferred or ability to obtain prior to start
