Responsibilities
- Provide direct patient care to residents under the supervision of a registered nurse or physician, supporting activities of daily living and comfort.
- Administer medications and treatments per state regulations and facility policy, with accurate documentation in patient records.
- Monitor vital signs, assess changes in condition, and promptly report findings to the supervising nurse.
- Assist with care planning, implementing interventions, and adjusting care as residents’ needs evolve.
- Promote safety, infection control, and resident dignity; support mobility, nutrition, and hygiene needs.
- Collaborate with an interdisciplinary team to coordinate care and respond to changes in clinical status or emergencies.
Requirements
- Active Licensed Practical Nurse license in Vermont (in good standing)
- BLS (American Heart Association) certification required or as specified for the assignment
- Graduation from an accredited practical nursing program
- Minimum of relevant experience in long-term care or skilled nursing settings preferred
- Ability to work in a long-term care/assisted living setting with a flexible schedule
- Strong communication, documentation, and teamwork skills
