Guaranteed hours: 72 hours per 2-week pay period
Shift: 5x8 hour Days
Setting: Level II trauma center with multiple inpatient units

Responsibilities
- Facilitate discharge planning and care coordination for patients across the acute care continuum
- Collaborate with the interdisciplinary team, including Social Services, to address barriers to discharge
- Manage a caseload of approximately 25–30 patients with a focus on safe, timely transitions of care
- Document all activities and plans in the EPIC charting system; ensure compliance with discharge planning protocols
- Coordinate care across units and campuses to optimize patient flow and outcomes
- Participate in interdisciplinary rounds and contribute to care plans from admission through discharge

Requirements
- Active RN license (or MSW for social worker hires); BLS certification
- Minimum 1 year acute care experience required
- EPIC charting experience or strong EPIC proficiency
- Ability to float to like units and between two nearby campuses (<2 miles apart)
- Availability to work 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, NYE, New Year’s Day)
- RTO requests to be provided at submittal per policy
- Locals not accepted
