This is a travel temporary assignment. The contract includes every other weekend.

Responsibilities
- Administer prescribed medications and treatments per the care plan and protocol
- Monitor vital signs, assess patient status, and document findings in the clinical record
- Assist with activities of daily living, mobility, hygiene, and nutritional support
- Collaborate with RNs, physicians, and other clinicians to implement and adjust care plans
- Maintain accurate documentation in the electronic health record; ensure safety and infection control standards
- Provide patient and family education as appropriate and support treatment plan adherence
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Current Basic Life Support (BLS) certification
- Prior LPN experience in long-term care or related behavioral health settings preferred
- Ability to work day shift (7:00 am–3:30 pm) with every other weekend required
- Strong communication and documentation skills; proficient with electronic health records
- Ability to work effectively within a multidisciplinary care environment
