Physical Therapy Assistant – LTAC in Fair Lawn, New Jersey. This travel assignment starts on 07/20/2026 for 13 weeks, with guaranteed 35 hours per week. Work a 5x8 day schedule (08:00–16:30) in a skilled nursing facility with long-term acute care needs. This role centers on delivering rehabilitative services to residents under supervisory guidance, supporting mobility, independence, and functional outcomes.

Responsibilities
- Provide Physical Therapy services under supervision per plan of care for LTAC residents
- Assess and re-evaluate functional status; document progress and adjust interventions
- Lead therapeutic exercises, gait training, ROM, balance activities, ADLs, and transfers
- Apply modalities and manual techniques per protocol; ensure patient safety and comfort
- Collaborate with PT, nursing, rehab, and case management; participate in rounds and care planning
- Maintain accurate records in the EMR; uphold infection control, safety, and privacy standards
Requirements
- Active Physical Therapy Assistant license in New Jersey or eligibility for licensure
- Associate degree from an accredited PTA program
- Ability to work a 5x8 schedule and travel to assignment as required
- Experience in LTAC or skilled nursing settings preferred
- Strong communication and teamwork skills; reliable transportation to the assignment
