Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of an RN.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions; involve the resident/family and the healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate referrals to ensure comprehensive care.
- Maintain current knowledge and competency in nursing practice; participate in mentoring and orientation of staff; demonstrate leadership within the professional practice setting as directed by the RN.
- Adhere to safety standards and infection control practices; manage care for adult and geriatric residents with attention to bloodborne pathogens and confidential information.
Requirements
- NY Licensed Practical Nurse licensure (active)
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Availability for every other weekend and holidays as needed
- Ability to provide care to adult and geriatric residents and to work under RN supervision
