Responsibilities
- Administer skilled nursing care to patients in their place of residence under the supervision of an RN and according to the plan of care.
- Coordinate care with the interdisciplinary team and communicate with patient/family; document outcomes and update clinical records accordingly.
- Assist in planning, coordinating, and implementing patient care to ensure quality, cost-effective services.
- Provide patient and family education on self-care techniques; assess responses to care and adjust interventions as needed.
- Maintain infection control practices and safety standards in all patient encounters.
- Assist the RN/physician with specialized duties, including end-of-life care when applicable, and support admissions, discharges, and transfers.
- Participate in on-call duties if required and ensure timely communication of patient status to the care team.
- Utilize electronic medical charting and time-keeping software (Homecare Homebase) for accurate documentation.
Requirements
- Active Licensed Practical Nurse (LPN) license in the practicing state (Virginia).
- Current CPR certification.
- Two years of LPN/LVN clinical experience, preferably in home health or hospice settings.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Valid driver’s license with reliable transportation; current auto liability insurance.
- Ability to read, write, and speak fluent English.
- Proficiency with Homecare Homebase or willingness to learn electronic medical charting and time-keeping software.
- Proof of hepatitis consent/declination and TB vaccination record.
- Ability to work in a home healthcare setting and adhere to professional boundaries and safety standards.
