Responsibilities
- Administer skilled nursing care to patients in their place of residence as prescribed by the physician and per the established plan of care.
- Work under the supervision of an RN, coordinate care with the interdisciplinary team, and communicate with patient/family. Document outcomes and report to the assigned RN Case Manager.
- Participate in planning, coordinating, and implementing patient care with the RN to provide quality, cost-effective services; maintain clear clinical notes and communications.
- Provide patient/family education on self-care techniques and assess response to care; ensure infection control measures protect staff and patient.
- Assist the RN and/or physician with specialized duties, including end-of-life care for Hospice patients when applicable; assist with admitting, discharging, and transferring patients.
- Potential on-call duties as required by the assignment.

Requirements
- Current LPN/LVN license in the practicing state (Virginia).
- Current driver's license with valid auto liability insurance.
- Current CPR certification.
- Two years of LPN/LVN experience in a clinical setting, preferably in home health or hospice.
- Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Proof of Hepatitis consent/declination and TB vaccination record.
- Ability to use electronic medical charting and time-keeping software (Homecare Homebase).
- English fluency; reliable attendance; ability to maintain professional boundaries in interactions with patients, families, and co-workers.
- Must be able to work in a home healthcare setting and travel as required by the role.
