Responsibilities
- Conduct comprehensive OT evaluations for residents in the long-term care setting and determine appropriate goals
- Develop and implement individualized treatment plans to improve activities of daily living, mobility, cognition, and functional independence
- Monitor and document patient progress, perform re-evaluations, and adjust plans of care as needed
- Collaborate with an interdisciplinary team (nursing, therapy, rehab, social services) to coordinate care and discharge planning
- Educate residents and families on therapy goals, routines, and home exercise/activity programs
- Ensure compliance with regulatory standards, documentation requirements, and safety protocols
Requirements
- Current Occupational Therapist license in the United States
- 1+ year of OT experience, with preference for acute care or skilled nursing/long-term care settings
- Ability to work effectively in a travel assignment and collaborate within a multidisciplinary team
- Strong communication, documentation, and patient-centered care skills
- Flexibility to work a daytime schedule in a long-term care facility
- Capacity to operate in a 13-week assignment with adherence to facility policies
