Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans
- Implement therapy programs focused on mobility, strength, balance, gait, and functional independence
- Conduct therapeutic exercise, gait training, ROM, manual therapy, and functional activities as appropriate
- Reassess progress, document findings, update care plans, and communicate with the interdisciplinary team
- Educate residents and families on exercises, safety, fall prevention, and home maintenance post-discharge
- Maintain accurate documentation, ensure compliance with regulatory and facility policies, and participate in care plan meetings
Requirements
- Eligible to work in the United States with ability to meet state licensure/credentialing requirements as needed
- Availability to start on 05/25/2026 and commit to a 13‑week travel assignment
- Ability to work 35 hours per week on a 5x8 day shift
- Experience or comfort working in a long‑term care setting and collaborating with an interdisciplinary team
- Strong communication, documentation, and interpersonal skills
- Ability to adapt to a travel assignment environment and comply with facility policies
