Responsibilities
- Deliver coordinated nursing care for a patient or designated group of patients according to established standards of care and the nursing process.
- Assess patient needs, implement care plans, administer medications and treatments, and monitor patient responses.
- Document care delivery and communicate changes to the interdisciplinary team to ensure continuity of care.
- Supervise and direct activities of various levels of assigned nursing staff on the shift.
- Coordinate care with other disciplines to support comprehensive, patient-centered plans.
- Apply critical thinking and professional judgment in the delivery of nursing care and when delegating tasks.
Requirements
- Current RN license in California (or eligibility to practice in the state)
- Basic Life Support (BLS) certification or ability to obtain
- Graduation from an accredited nursing program (ADN or BSN)
- Experience in hospice or home health settings preferred
- Ability to work Nights, 12-hour shifts, in a home-based care environment
