Responsibilities
- Administer prescribed medications and treatments within approved nursing protocols.
- Monitor residents for changes in health status; take vital signs and report concerns promptly to the supervising RN or physician.
- Assist with activities of daily living and hygiene, including mobility support, dressing, bathing, and toileting.
- Perform basic nursing procedures and wound care as defined by the resident’s care plan and under supervision.
- Document assessments, interventions, and responses in the resident record; maintain accuracy, privacy, and compliance with regulatory requirements.
- Collaborate with the interdisciplinary team to implement and update individualized care plans; adhere to safety and infection-control standards.

Requirements
- Active Licensed Practical Nurse license in good standing.
- Ability to work night shifts; 12-hour shifts with 36 guaranteed hours per week.
- Completion of an approved LPN program (diploma) and related training.
- Experience in a long-term care or skilled nursing setting preferred.
- Ability to pass background check and drug screen; basic documentation and charting proficiency.
- Good communication skills and ability to collaborate with the care team.
