Responsibilities
- Process insurance authorizations and pre-certifications for cases in the case management workflow
- Coordinate post-acute referrals and support discharge planning with care teams
- Deliver Medicaid notices and assist with benefits eligibility related tasks
- Document care coordination activities in Epic and support clerical case-management processes
- Facilitate referrals to home health, DME, rehab, long-term care, and related services
- Support scheduling, interpreter services, and transportation coordination as part of the care continuum
Requirements
- Epic charting experience required
- Minimum 1 year of relevant experience; clinical background helpful but not required
- Licensure: Washington license not required for this role
- Locals not accepted
- Ability to work in a case-management clerical setting; on-site required
- References: 2 references covering 1 year within the last 3 years
- Background/Nursys: disclose any background or Nursys issues at submittal
- Holidays: must commit to working 3 of the 5 listed holidays (Thanksgiving, Christmas Eve, Christmas Day, NYE, NYD)
