Responsibilities
- Assist the supervising physical therapist in delivering the plan of care for LTAC patients
- Implement therapeutic exercises, gait training, balance activities, ROM, and functional mobility tasks
- Monitor patient responses, document progress, and communicate changes to the PT and care team
- Support safe patient handling, transfers, positioning, and daily therapeutic activities
- Educate patients and families on prescribed exercises, activity plans, and safety measures
- Collaborate with nursing staff, therapy aides, and other clinicians to ensure cohesive patient care
Requirements
- Ability to work in a LTAC/SNF setting
- Strong communication and teamwork skills
- Willingness to travel for a temporary assignment
- Availability to work the posted shift schedule
- Ability to meet the physical demands of therapy services and maintain safety protocols
