Role Specifics and Benefits:
- Core responsibilities: Provide skilled nursing care to residents in long-term care settings under supervision, including medication administration, monitoring vital signs, wound assessment and care, insulin administration and diabetes management, routine assessments, care planning, and timely documentation. Assist residents with activities of daily living, mobility and falls prevention, nutrition support, and rehabilitation therapies as coordinated with the broader care team. Recognize and report changes in condition promptly, participate in care conferences, and uphold infection control and safety standards. Collaborate with physicians, therapists, social workers, and family members to ensure holistic, person-centered care and smooth transitions between care phases.
