Registered Nurse – Home Health (Hospice) in Newport, Vermont. Travel assignment through Healthforce Healthcare LLC. Start date: 09/21/2026 for approximately 25 weeks. Weekly pay: $1,721–$1,766. Guaranteed hours: 32 per week. Shift: 5x8 Days (Monday–Friday, 8:00 a.m.–5:00 p.m.) with some weekend coverage; on-call may be scheduled only on an emergency basis. Setting: Home health and hospice care.

Responsibilities
- Provide skilled nursing care to patients in their homes, including assessments and ongoing monitoring
- Perform OASIS data collection and documentation within the HealthWyse (Careficient) EMR
- Administer medications, manage therapies, and implement individualized care plans
- Coordinate with an interdisciplinary team to adjust care as patient needs change
- Educate patients and family caregivers on disease processes, medications, safety, and self-care
- Travel between patient homes for scheduled visits and respond to urgent needs as assigned while maintaining accurate documentation
Requirements
- Active Registered Nurse license in Vermont
- OASIS experience
- Home Health and Hospice experience
- Experience traveling between patient homes for home visits; ability to manage schedule changes
- Valid driver’s license in good standing and reliable transportation
- BLS certification and proficiency with HealthWyse (Careficient) EMR (or willingness to learn)
