Responsibilities
- Deliver coordinated nursing care for a patient or an assigned group of patients according to established standards of care and the nursing process.
- Supervise and direct activities of various levels of assigned nursing staff; coordinate care with other disciplines.
- Monitor patient status, document care, assess for changes, and implement appropriate interventions.
- Communicate with patients and families; provide education and participate in discharge planning as appropriate.
- Maintain safety, infection control, and regulatory compliance in a home hospice setting.
- Collaborate with the interdisciplinary team to ensure continuity of care and effective care planning.
Notes: This department only includes education and non-productive hours of Float Pool nurses. Float Pool nurses working a unit should always clock in to the unit being worked.
Requirements
- Active Registered Nurse license (state of practice)
- Ability to work in a home healthcare - hospice environment
- Availability for 12-hour night shifts
- Strong clinical assessment, critical thinking, and communication skills
- Ability to document and coordinate care within established standards
- Eligibility to work in the United States
