Responsibilities
- Assess residents’ PT needs and develop individualized treatment plans aligned with prognosis and goals.
- Implement therapeutic exercises, gait and balance training, mobility optimization, and pain management strategies.
- Document evaluations, treatment plans, progress notes, and discharge planning in the patient record; communicate with the interdisciplinary care team.
- Monitor and adjust plans based on patient response; ensure safety and fall-prevention measures during therapy sessions.
- Educate residents and families on home exercise programs, equipment use, and strategies to promote independence.
- Collaborate with other therapy disciplines, nursing, and rehabilitation staff to optimize resident outcomes and ensure regulatory compliance.

- Ability to work in a skilled nursing facility setting
- Availability for a 13-week travel assignment starting 05/11/2026
- Commitment to a 35-hour guaranteed workweek
- Strong documentation and communication skills
- Willingness to collaborate with an interdisciplinary care team
- Ability to adhere to a 5x8 day shift schedule
