Licensed Practical Nurse (LPN) – Long-Term Care travel assignment in Rutland, Vermont. Start date June 22, 2026 for an assignment measured in weeks with 40 guaranteed hours per week on a consistent 2:00 PM–10:00 PM shift. Responsibilities
- Provide direct, hands-on nursing care to residents in a long-term care setting, consistent with individualized care plans.
- Administer medications and treatments as prescribed, monitor effects, and document responses in the chart.
- Monitor vital signs, assess changes in resident condition, and report concerns promptly to the supervising RN or physician.
- Assist residents with activities of daily living, including ADLs, mobility, nutrition, and personal hygiene.
- Maintain accurate documentation, ensure infection control practices, and uphold safety and resident-rights standards.
- Collaborate with an interdisciplinary team to support care planning, patient comfort, and timely changes in care as needed.
Requirements
- Active LPN license in good standing.
- Education: graduate of an accredited practical nursing program.
- Experience: experience in a long-term care or skilled nursing setting preferred.
- Availability to work the 14:00–22:00 shift and to fulfill guaranteed weekly hours.
- Strong communication and documentation skills; ability to collaborate with a diverse care team.
