Responsibilities
- Provide skilled nursing care to residents according to individualized care plans.
- Administer prescribed medications and treatments under supervision of licensed clinicians.
- Monitor residents’ vital signs and overall condition; document changes accurately and promptly.
- Assist with activities of daily living (ADLs) and mobility to support resident independence and safety.
- Communicate changes in resident status to the supervising RN and interdisciplinary team; participate in care plan updates.
- Maintain thorough, accurate documentation and uphold infection control and safety practices.
Requirements
- Active LPN license and ability to practice nursing in good standing.
- Availability for night-shift work (3x12-hour shifts) and on-site coverage as needed during the assignment.
- Ability to work effectively in a long-term care setting with older adult residents.
- Strong clinical documentation, observation, and communication skills.
- Commitment to safety, patient advocacy, and teamwork in a supportive care environment.
