Responsibilities
- Conduct comprehensive PT evaluations for residents in a geriatric/long-term care setting and develop individualized treatment plans.
- Implement evidence-based interventions to improve mobility, gait, balance, strength, ROM, transfers, and activities of daily living.
- Monitor progress, document outcomes in the patient record, and update plans of care in coordination with the interdisciplinary team.
- Educate residents and families on home exercise programs, safety strategies, fall prevention, and activity modification.
- Collaborate with nurses, physicians, and other rehabilitation teammates to optimize discharge planning and continuity of care.
- Ensure compliance with regulatory requirements, facility policies, and infection control practices; maintain accurate and timely documentation.

Requirements
- Active Physical Therapist license in New Hampshire (or eligibility for licensure in NH)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Minimum 1 year of physical therapy experience in skilled nursing, geriatric, or similar setting preferred
- Ability to perform full-time hours on a 5x8 daytime schedule (08:00–16:30) and to work with geriatric patient population
- Strong documentation skills and proficiency with electronic health records
- CPR/BLS certification or equivalent, and good communication skills with interprofessional teams
