- Core responsibilities: Conduct comprehensive in-home assessments; develop, implement, and adjust patient-centered care plans in collaboration with physicians and families; provide symptom management, wound care, and medication administration per orders; deliver patient and caregiver education on safety, disease management, and adherence; document care activities in the electronic health record; coordinate referrals to rehabilitation, social work, and other services; support discharge planning and transitions to community-based resources; uphold infection control and safety standards.
