Responsibilities
- Conduct initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations.
- Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and promote independence.
- Deliver direct therapy interventions including therapeutic exercises, gait training, balance retraining, and the use of modalities as ordered.
- Monitor resident progress and adjust care plans; document treatment sessions, progress notes, and assist with timely billing.
- Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements.
- Participate in interdisciplinary care team meetings and contribute to discharge planning; educate residents and families on home exercise programs and safety.
Requirements
- Active Physical Therapy license in Missouri (state of practice)
- Graduate of an accredited Physical Therapy program
- Experience in SNF or geriatric rehabilitation preferred
- Strong clinical, communication, and organizational skills
- Ability to work independently and as part of an interdisciplinary team
- Willingness to supervise PTAs and therapy aides as allowed by licensure
Education
- Graduate of an accredited Physical Therapy program
