Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients in a home hospice setting according to established standards and the nursing process
- Collect and analyze health data, assess needs, and develop a care plan; implement plan and coordinate care with the interdisciplinary team
- Administer medications and treatments per physician orders; monitor for response and adverse events
- Supervise and direct activities of various levels of nursing staff as assigned; ensure safe, effective care delivery
- Educate patients and families on disease management, symptom relief, medication purposes, and discharge planning
- Document care activities in the medical record; participate in ongoing evaluation of patient progress and discharge planning

Requirements
- RN licensure: Active, in the state of practice (California)
- Certifications: Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) required
- Additional certifications: Advanced Cardiovascular Life Support (ACLS) required per policy
- Experience: At least two years of recent nursing experience in the field; EPIC electronic medical record experience required
- Education: Bachelor’s Degree in Nursing (BSN) preferred
- Setting: Ability to work in a home healthcare – hospice environment; night shift availability as scheduled
