Registered Nurse – Case Manager in Grand Junction, Colorado. This travel, temporary assignment starts on 04/13/2026 and runs for 13 weeks in an outpatient urgent care setting. Responsibilities
- Manage an assigned patient caseload and lead the interdisciplinary team in coordinating care for those patients.
- Assess patient needs, develop, implement, and revise individualized care plans with clear goals of care.
- Educate patients and families, support caregivers, and facilitate patient and family understanding of treatment options.
- Facilitate referrals, care transitions, and connections to community resources to support safe and effective care.
- Document in the electronic health record, maintain accurate records, and ensure compliance with applicable standards and regulations.
- Collaborate with physicians, social workers, and other care team members to optimize outcomes and resource use.
Requirements
- Active Registered Nurse license in Colorado.
- 1+ year of RN experience; case management or care coordination experience preferred.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work in an outpatient/urgent care setting with a weekday daytime schedule (Mon–Fri, 8:00 a.m.–5:00 p.m. typical).
- Proficiency with electronic health records and Team-based collaboration; ability to work both independently and as part of a team.
- Education: graduation from an accredited nursing program (ADN or higher); BSN preferred.
- Certifications: BLS certification preferred (vendor requirements may vary).
