Responsibilities
- Administer prescribed medications and treatments under supervision, documenting delivery according to policy.
- Perform routine assessments and ongoing monitoring of residents, identifying changes and communicating with the care team.
- Provide direct patient care, including assistance with activities of daily living and mobility, while supporting residents’ comfort and safety.
- Participate in care planning and interdisciplinary rounds, updating care plans as needed and ensuring continuity of care.
- Document accurately in the electronic medical record, maintaining HIPAA-compliant patient confidentiality.
- Respond to call lights promptly, assist with wound care and dressing changes as prescribed, and monitor vital signs and input/output as needed.

Requirements
- Active LPN license (New Hampshire) or eligibility for licensure in the state.
- Current CPR/BLS certification.
- Prior experience in long-term care or skilled nursing settings preferred.
- Ability to work evenings on a 5x8 schedule and to accommodate local-week restrictions as described.
- Completed LPN diploma from an approved program; maintain ongoing professional development as required.
