Responsibilities
- Deliver coordinated, patient-centered nursing care in a home healthcare - hospice setting according to established standards of care and the nursing process.
- Collect relevant patient data, analyze assessment findings, and develop a care plan with prescribed interventions to achieve outcomes.
- Implement the plan, coordinate care delivery, and collaborate with interdisciplinary team members to promote safety and healing.
- Supervise and direct activities of various levels of nursing staff, delegate appropriately, and oversee care coordination across disciplines.
- Participate in discharge planning and continuity of care, provide health information to patients, families, and the treatment team, and support patient/family education.
- Maintain accurate documentation, ensure a safe environment, and clock in to the unit being worked for float pool assignments.
Requirements
- Active Registered Nurse license in the state of California
- ICU or critical care nursing experience; float pool experience preferred
- Ability to work in a home healthcare - hospice setting and participate in a 13-week travel assignment
- Strong communication, critical thinking, and teamwork skills; comfort with coordinating care across disciplines
- Ability to clock in to the unit being worked and maintain clear handoffs to ensure continuity of care
- Commitment to patient safety and adherence to organizational policies, including vaccination requirements and right-to-work compliance
