Responsibilities
- Conduct comprehensive OT evaluations for residents and develop individualized treatment plans to address ADLs, mobility, cognition, and safety
- Deliver direct therapy services and implement therapeutic activities aimed at improving functional independence
- Reassess resident progress, adjust treatment plans, and document outcomes in medical records per regulatory standards
- Collaborate with interdisciplinary team members (nursing, PT, SLP, social work) to coordinate care and support discharge planning
- Ensure equipment and adaptive devices are appropriately selected, applied, and monitored
- Maintain compliance with documentation, patient safety, and facility policies; participate in quality improvement initiatives as appropriate
Requirements
- Active OT license in the United States with eligibility to practice in Maryland (or standard licensure pathway to Maryland)
- Experience in or comfort with a skilled nursing/long-term care setting preferred
- Strong documentation and communication skills; ability to work within an interdisciplinary team
- Demonstrated ability to manage a caseload and adapt to a travel assignment environment
- Ability to meet the duties of a 35-hour per week schedule and engage residents across a SNF setting
