Responsibilities
- Conduct initial and ongoing physical therapy assessments for residents to identify treatment needs and goals.
- Develop, implement, and adjust individualized therapy plans focusing on mobility, gait, balance, strength, range of motion, and functional outcomes.
- Deliver evidence-based PT interventions and monitor resident progress, adjusting plans as needed.
- Document evaluations, treatment sessions, clinical findings, and progress notes in the facility’s electronic health record; maintain accurate and timely records.
- Collaborate with the interdisciplinary team (nursing, physicians, case management, therapy assistants) to coordinate care, discharge planning, and transition to home or community settings.
- Educate residents and families on prognosis, home exercise programs, fall prevention, and safety strategies; ensure patient safety and adherence to care plans.
Requirements
- Active Physical Therapist license in Maine or evidence of eligibility for licensure (state licensure by endorsement acceptable)
- Doctor of Physical Therapy (DPT) degree from an accredited program
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
- Current CPR/BLS certification or ability to obtain prior to start
- Ability to work in a skilled nursing facility environment, follow clinical protocols, and collaborate with the care team
