Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for residents in a SNF setting.
- Deliver evidence-based therapies to improve mobility, balance, gait, strength, and functional independence.
- Monitor progress, adjust plans as needed, and document patient outcomes in the medical record.
- Collaborate with an interdisciplinary team (nursing, therapy aides, physicians, and others) to optimize resident care and discharge planning.
- Educate residents and families on home exercise programs, safety strategies, and activity modification to promote independence.
- Maintain compliance with facility policies, documentation standards, and patient safety protocols.
Requirements
- Doctor of Physical Therapy (DPT) from an accredited program.
- Active Physical Therapist license in the United States or eligibility for licensure by endorsement/PA licensure.
- Ability to practice in a skilled nursing facility setting and commit to the 13-week contract.
- Willingness to travel for temporary assignments and adapt to varied patient needs.
- Strong communication, documentation, and teamwork skills; ability to work independently when needed.
- Prior SNF or long-term care experience is preferred.
