Travel Registered Nurse Case Management Job
AZ-Clinic Care Coordinator-RN-DAYS — Registered Nurse, Case Management is available for a travel assignment in Fort Defiance, Arizona. Start date: 04/06/2026. This is a 13-week travel assignment with guaranteed 36 hours per week. Shift: 5x8 Days. Setting: Short Term Acute Care Hospital outpatient surgery clinic. Responsibilities center on clinic-based case management coordination in the outpatient surgical environment.

Responsibilities
- Coordinate patient care within the Surgery Outpatient Clinic, collaborating with nursing staff and providers
- Develop, implement, and monitor individualized care plans; arrange referrals and post-discharge follow-up
- Communicate with patients and families to support understanding of care plans, discharge instructions, and available resources
- Document care coordination activities accurately in the medical record; ensure continuity of care across settings
- May assist with triage, answer patient questions, and support workflow; may float to other departments as needed
Requirements
- Active Registered Nurse license
- Three (3) years of Case Management experience
- Ability to work in a clinic/outpatient setting; Monday–Friday, 8:00 am–5:00 pm; one-hour lunch; no weekends or holidays
- Strong communication, documentation, and care coordination skills
- Orientation requirements: New Employee Orientation scheduled every other Monday; start date may be adjusted if compliance deadlines are not met
- Willingness to float to other departments as needed
