Responsibilities
- Provide direct, compassionate nursing care to residents in a skilled nursing environment per the approved plan of care
- Administer medications and treatments in accordance with physician orders and facility policies
- Monitor, assess, and document patient conditions; report changes to the supervising RN promptly
- Assist residents with activities of daily living, including hygiene, mobility, and comfort measures
- Maintain accurate and timely documentation in the facility’s electronic health records
- Collaborate with the interdisciplinary team to implement and adjust care plans; ensure patient safety and infection control standards
- Respond to call bells and emergencies in a calm, professional manner; uphold resident dignity and safety
Requirements
- Current LPN license to practice in Vermont or eligibility to obtain licensure
- Basic Life Support (BLS) certification
- Ability to work a 40-hour week, including weekends, on a 7:00–15:30 schedule
- Competence in essential nursing duties within a skilled nursing setting
- Strong communication skills and ability to collaborate with a care team
- Prior experience in long-term care or skilled nursing care preferred
