Responsibilities
- Perform initial and ongoing physical therapy evaluations to determine residents’ functional status and rehab needs
- Develop and implement individualized treatment plans addressing mobility, balance, gait, strength, and endurance
- Deliver therapeutic interventions including therapeutic exercises, manual therapy, and modality applications as clinically appropriate
- Reassess resident progress regularly, adjust treatment plans, and document findings in the medical record
- Collaborate with nursing, therapy colleagues, and the broader care team to coordinate goals and discharge planning
- Educate residents and families on home programs, safety, fall prevention, and activity modification
Requirements
- Active Physical Therapist license in Washington or eligibility for licensure transfer
- Current CPR/BLS certification
- Ability to work in a skilled nursing facility setting and manage a daytime 8-hour shift
- Strong documentation and communication skills
- Experience in geriatric rehabilitation or SNF settings preferred
- Willingness to participate in a travel assignment lasting several weeks
