Responsibilities
- Provide skilled nursing care to patients in a home hospice setting, including assessments, monitoring, medication administration, and wound care per the plan of care.
- Manage acute symptoms, respond to changing patient conditions, and escalate concerns as needed.
- Document care accurately in the electronic health record and maintain timely, compliant charting.
- Educate patients and families on disease process, symptom management, medications, and safety measures.
- Collaborate with the interdisciplinary team to coordinate patient care and communicate changes to supervising clinicians.
- Follow unit policy for float pool assignments; float pool nurses on a unit should clock in to the unit being worked.
Requirements
- ER or acute care nursing experience preferred
- Ability to work 12-hour night shifts
- Experience in home healthcare or hospice settings is a plus
- Strong clinical assessment, decision-making, and documentation skills
- Effective communication and teamwork in a travel assignment
- Ability to adapt to different patients and care settings as part of float pool coverage
