Physical Therapy Assistant – LTAC, Canaan, CT
This travel assignment places you in a long-term acute care setting for 13 weeks, starting 08/31/2026. You will work under a licensed PT to deliver rehabilitation services to patients, with guaranteed 35 hours per week and 5x8-style shifts. The weekly pay range is $1,586–$1,672. The work environment is a LTC - Nursing Home setting that supports restorative therapy within a multidisciplinary team.

Responsibilities
- Deliver physical therapy services under the supervision of a licensed PT, including therapeutic exercises, functional training, gait and balance activities.
- Implement and adjust components of the plan of care, select appropriate modalities, and monitor patient responses.
- Document interventions, progress notes, and outcomes accurately in the patient record; communicate changes to the supervising PT and care team.
- Support patient safety during mobility, transfers, and daily activities; collaborate with nursing and rehabilitation staff for coordinated care.
- Educate patients and families on home exercise programs, safety strategies, and progression toward rehabilitation goals.

Requirements
- Active Physical Therapy Assistant license in Connecticut
- Ability to work in a LTAC/long-term care setting
- Availability for a 13-week travel assignment and guaranteed 35 hours per week
- Willingness to work a schedule of 08:00–16:30 (5x8) shifts
- Strong communication, teamwork, and reliability to support interdisciplinary care
