Responsibilities
- Evaluate and treat adults with speech, language, communication, cognitive-communication, and swallowing disorders within a SNF setting.
- Develop, implement, and monitor individualized therapy plans; adjust goals based on progress and discharge planning.
- Document evaluations, progress notes, treatment plans, and discharge summaries in the patient chart per facility policy and regulatory requirements.
- Collaborate with an interdisciplinary team to optimize comprehensive patient care and participate in care planning meetings.
- Provide education and support to patients, families, and caregivers regarding communication strategies and therapy goals.
- Ensure adherence to safety, infection control, and documentation standards during all therapy sessions.
Requirements
- Active Georgia state license or eligibility to practice as a Speech Language Pathologist.
- Ability to work on-site in a skilled nursing facility setting.
- Strong communication, documentation, and patient care skills.
- Eligibility to work in the United States.
- Experience in adult populations or skilled nursing facilities is desirable but not strictly required.
