Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups.
- Implement and monitor individualized treatment plans in collaboration with the care team.
- Document patient progress, adjust plans as needed, and support care planning and discharge planning.
- Provide patient and family education on therapy goals, progress, and home programs.
